THIS APPLICATION IS NOT COMPLETE UNTIL YOUR SUPPORTING DOCUMENTATION LISTED BELOW IS RECEIVED. Failure to provide all requested documentation within 10 calendar days will result in your incomplete application not being processed. Required supporting documentation for all applicant households:

Social Security cards for all family members

Adults: An official birth certificate for each household member or naturalization papers (for non-U.S. birth certificates) ; AND one of the following: A current state issued photo I.D. (such as a driver license), U.S. Military Discharge (DD 214), U.S. Passport, other government issued identification card

Children: An official birth certificate; or U.S. Passport or other government issued identification card

Photocopies of documents listed above are accepted to determine initial eligibility, however, the Sioux City Housing Authority requires you to provide your original social security cards and birth certificates to be viewed and copied prior to final eligibility determination.

Documents to complete your application are accepted in person Monday-Friday from 8:00 a.m. to 3:30 p.m. You may submit the required documentation in person, or by mail. Faxes are not accepted. At this time, the Housing Authority is not accepting applications or accompanying information by email due to security risks. Please do not mail originals.
