- Date of Birth for all Household Members
- Race and Ethnicity for all Household Members Note: Race and Ethnicity information is being collected for statistical purposes only and does not impact your eligibility for the Housing Choice Voucher Program.
- Valid E-Mail Address
ATTN: As a part of this process, you will need to create an account using a valid e-mail address or use a WaitListCheck account you already have.
